Custom Error Pages not working

Just discovered that my custom error page 404.shtml is not kicking in as it should and instead visitors are receiving the standard error page.

Its seems to be happening on all my domains not just one

Will copy this to support also.

Thanks for spotting this Brian, I see I have the same problem. It used to work, and if I go to cPanel and select Error pages I can still see my code, but saving it again doesn't seem to fix it.
